Documentation & SEVIS
- DS-2019 form issued — SEVIS ID assigned — 22 CFR 62.13
- SEVIS fee paid ($35) — confirmed via I-901 receipt
- Visa interview — student attended embassy interview
- J-1 visa approved — copy on file
- Program fee payment — documented with receipt
Insurance Verification (22 CFR 62.14)
- Health insurance policy — meets minimum coverage requirements:
- Medical benefits: $100,000 per accident/illness
- Repatriation of remains: $25,000
- Medical evacuation: $50,000
- Deductible: $500 or less
- Insurance card copy — on file with policy number and provider contact
- Coverage dates — verified to span entire program period
During-Program Checklist
Monthly Requirements
- Local Coordinator monthly report — submitted for each student — 22 CFR 62.25(g)
- Student wellbeing assessment
- Host family relationship status
- Academic progress check
- Any concerns or incidents
- Contact verification — LC has made contact with student AND host family
Semester Requirements
- Semester activity report — comprehensive mid-year/end-of-year review
- School academic report — grades and attendance from host school
- Insurance renewal verification — if policy expires mid-program
As-Needed Documentation
- Incident reports — any safety concerns documented immediately
- Host family change — full re-vetting required for new family (repeat 22 CFR 62.25(j))
- School change — documented with reason and new school confirmation
- Early return — SEVIS record updated, reason documented
Post-Program Checklist
- SEVIS record updated — program completion status entered
- Student departure confirmed — travel itinerary and confirmation on file
- Final evaluation — student feedback collected
- Host family evaluation — host family feedback collected
- File archived — all documents retained for minimum 5 years
